On this page you'll find application forms relating to Lump Sum Allowance and decisions about PPF
entitlement and overpayments. Each application form is downloadable straight to your computer.
With the exception of the Lump Sum Allowance form, all forms can be filled in online although
you'll need to print and sign the forms manually before returning them to us either online or by
post. If you'd like to send the form to us online, please register and log in to this site. Once
logged in, you can find our 'Send us a document' facility under the 'Communications' tab in the
main menu.
